For your convenience, we've provided our patient forms on our website. To help expedite your first visit, please download, print, and complete the forms. Bring them with you when you come in for your first visit. If you choose to fill out the patient form packet at our office, please arrive 10-15 minutes before your appointment to allow enough time to get them complete before your appointment.​

Do I have to be a member of the PACIFIC CLINIC to receive services from Physical Therapy @ The Pacific Clinic?  

No! Not required, but recommended. To ensure that proper healing takes place. During your PT, you will also have access to The Pacific Clinic with a 1-month Wellness Basic Individual Membership.

When my doctor recommends therapy, do I have a choice in where I go?  

Yes, if you need physical therapy, YOU DO HAVE A CHOICE when selecting a provider.  Be sure to explore all your options.  After doing your research, we are confident you will see the value in choosing Physical Therapy @ The Pacific Clinic.

What insurance providers do you accept? 

We accept virtually all major insurance carriers. We verify and share your insurance benefits and requirements with you. 

What should I expect on my first visit?​

Your first visit is approximately one hour.  After reviewing your medical history, diagnostic tests and recent events that may have contributed to your current condition, your therapist will make an initial evaluation. The evaluation includes a full assessment of your current functional deficits, pain level and posture.  You also receive a detailed evaluation of your flexibility, strength, balance and endurance.Following the evaluation, your physical therapist explains your personalized treatment plan, reviews expectations for recovery and provides a commitment letter.

What should I bring to my first visit?

New patient paperwork or please arrive 15 minutes before your first appointment in order to complete all necessary paperwork

Will my insurance cover my therapy?​

​Physical Therapy @ The Pacific Clinic is contracted as a preferred provider for most insurance carriers.  We do our best to simplify the process by contacting your provider for details regarding your benefits and responsibilities.  After securing that information, we share it with you and answer your questions.

Whom should I contact with billing questions?​

Please call or text (509) 735-2014 during normal operating hours.  Our billing staff is happy to provide a thorough explanation of the billing process.
