Bring them with you when you come in for your first visit. If you choose to fill out the patient form packet at our office, please arrive 10-15 minutes before your appointment to allow enough time to get them complete before your appointment.

We accept virtually all major insurance carriers. We verify and share your insurance benefits and requirements with you.
Your first visit is approximately one hour. After reviewing your medical history, diagnostic tests and recent events that may have contributed to your current condition, your therapist will make an initial evaluation. The evaluation includes a full assessment of your current functional deficits, pain level and posture. You also receive a detailed evaluation of your flexibility, strength, balance and endurance.Following the evaluation, your physical therapist explains your personalized treatment plan, reviews expectations for recovery and provides a commitment letter.

Physical Therapy @ The Pacific Clinic is contracted as a preferred provider for most insurance carriers. We do our best to simplify the process by contacting your provider for details regarding your benefits and responsibilities. After securing that information, we share it with you and answer your questions.
